Referring Patients to HealthStyle

Patients can referred to one or more of HealthStyle’s clinicians by calling one telephone number - 4789 3425.

Patients are either privately billed with the potential for private health insurance, or may be eligible for Medicare or DVA rebates.

HealthStyle bulk-bills exercise physiology services for EPC referrals.

Medicare, DVA and Private Health Rebates

Patients with both an GP Management Plan (MBS item 721) and Team Care Arrangements (MBS item 723) are eligible for Medicare rebates for a total of five (5) allied health services per calendar year (five sessions per patient each year, not for each allied health provider). 

People with Type 2 Diabetes are also eligible for eight Medicare funded group services provided by Exercise Physiologists, Diabetes Educators, and Dietitians. These patients are required to have a GPMP in place and have a Type 2 Diabetes Allied Health Group Referral Form completed before being referred to one of the above health professionals for an individual assessment consultation.

Accredited Exercise Physiologists and Credentialed Diabetes Educators have joined other allied health professions in being eligible to provide services for holders of Department of Veterans Affairs Gold and White Card holders.

A range of private health insurers will provide rebates for our services - but patient must check their eligibility.

GP EPC Planning Service

HealthStyle’s GP EPC Planning service can improve patient outcomes, provide access to integrated multi-disciplinary healthcare teams, and increasepractice income - all without additional administrative or time stress.

HealthStyle staff apply evidence-based clinical practice guidelines for patients with diabetes, osteoporosis, obesity, and cardiovascular disease.  The paperwork burden for GPs and practice nurses is eliminated as HealthStyle does it all - a five minute GP-patient consult is al that's required to finalise the process.
